Parent/Guardian Release for Minors

I am the parent or guardian of ____________________, a child under 18 years of age who wishes to participate in hikes sponsored by the Triple Cities Hiking Club. I agree that members and affiliates of the Triple Cities Hiking Club and other participants in hikes sponsored by the Triple Cities are not acting as supervisors for my child, and are not responsible for providing any equipment for hikes or for my child’s physical condition. I understand that hiking, like other recreational activities and sports, can be inherently dangerous. Potential hazards while hiking include wild animals, poisonous plants, hazardous terrain, hunters, adverse weather conditions, and getting lost. On behalf of myself, my child, my spouse or other natural parent or guardian, and on behalf of my/our heirs, assigns, personal representatives and next of kin, I hereby release all members and affiliates of the Triple Cities Hiking Club and all other participants in hikes sponsored by the Triple Cities Hiking Club from all liabilities arising from my child's participation in hikes sponsored by the Triple Cities Hiking Club.

I also understand that it is my responsibility to provide contact information for each separate hike that my child participates in, and that my child's participation in a particular hike is entirely at the discretion of the leader that hike.
